About GCSCC

Here is the newly completed GCSCC office building. Grafton County Senior Citizens Council works throughout the region to make sure that older members of the community receive services that help them remain independent in their own homes for as long as possible.

The council serves nearly one out of every two residents over the age of 60. Over the last year, 6,601 people participated in the council's programs out of a county population of 14,323 individuals over the age of 60. The number of elderly people in Grafton County is growing, and the number of participants in our programs is also growing.

Incorporated in the Lebanon, New Hampshire area in 1973, the council has since assumed responsibility for service provision to older residents of all areas of the county. The council now has locations in Lebanon, Canaan, Orford, Woodsville/Haverhill, Plymouth, Bristol, Lincoln and Littleton, and provides a range of services and programs to older residents of 39 communities within Grafton County.

GCSCC provides additional services, including information & referral, recreation, education, adult day care, health screening, discount cards, chore/home repair service, telephone reassurance.

Grafton County Senior Citizens Council Endowment Fund, Inc.

Grafton County Senior Citizens Council (GCSCC) has formed an endowment fund to raise, invest and administer funds to support the goals of the council. The council works to provide community-based long-term care services for older residents of Grafton County. Such services support older residents' continued health and independence in their own homes and communities and facilitate their involvement in and with the community.

In support of the council's work and purposes, the trustees of the GCSCC Endowment Fund actively solicit gifts and administer a program of planned giving designed to increase the size of the fund. The trustees also meet donors' needs and objectives through their charitable giving. The trustees invest and manage financial assets to maximize support for elder services administered by GCSCC, and they provide oversight for the development functions of GCSCC.

The trustees have the goal of increasing the size of the Endowment Fund to $1,000,000 and to manage current assets to maximize investment income for support of elder services administered by GCSCC. Potential donors may obtain additional information by calling Roberta Berner, director of marketing and development, at the GCSCC office.

CONSUMER PRIVACY PROTECTION POLICY

As participants in programs offered by Grafton County Senior Citizens Council, Inc. and as a consumer of health and human services, consumers have the right to control all personal information gathered in the process of providing those services.

Information we collect:

  • Information we receive from consumers during the application for services
  • Information about services received by consumers from GCSCC
  • Information we receive from family members, physician, health care facility
Unless it is specifically stated otherwise, no other information will be collected from consumers.

Information we may disclose to third parties:

  • Information about the services consumers have received from GCSCC to NH Dept. Health & Human Services for the purpose of billing for services provided.
  • Information essential to obtaining services consumers have requested and which are provided by a third party, but only after having received a signed authorization.
  • Information to provide assistance to consumers in emergency situations.
We do not disclose any nonpublic personal information about GCSCC participants or former participants to anyone except as permitted by law.

GCSCC practices regarding information confidentiality and security:

  • We restrict access to nonpublic personal information about consumers to those employees who need to know information in order to provide services.
  • We maintain physical, electronic and procedural safeguards that comply with federal regulations to guard nonpublic personal information.

Reservations of the right to disclose information in unforeseen circumstances:

In the event that another service provider assumes the responsibilities of GCSCC for service provision to older and disabled adults residing in Grafton County, we reserve the right to transfer information (including but not limited to an address, name, age, gender, zip code and other information that consumers provide through communication with GCSCC staff) to a third party entity that: 1.) provides similar community based services to older and disabled residents of the area; 2.) agrees to be GCSCC’s successor with regard to maintaining and protecting the information collected; and 3.) agrees to the obligations of this privacy statement.

Consumers have the right:

  • To an explanation of how personal information may be used or disclosed
  • To control the release of personal information
  • To consent, in advance, prior to any release of health information
  • To see a copy of health records
  • To request a copy of health records
  • To request an amendment of health records
  • To obtain documentation of any disclosure of personal health information

Grafton County Senior Citizens Council, Inc. is mandated to comply with these requirements and:

  • To safeguard consumer personal information by keeping that information in locked files
  • To insure that computer files containing client records are password protected.
  • To establish and enforce policies restricting access to consumer records
  • To require written authorization from consumer prior to releasing any information, excepting in the case of medical emergency
  • To respond promptly to consumer requests for access to records
  • To amend information in consumer files in response to requests
  • To provide documentation of any disclosure of consumer personal health information
  • To comply with all other requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
