ServiceLink of Grafton County is one of ten Aging and Disability Resource Centers located across the state and may be reached toll free at 1-866-634-9412.
Each Center offers Medicare Counseling, Caregiver Counseling, Long-term Care Options Counseling and general information, referral and assistance for services in Grafton County. The Centers are free, accessible and open to the public. We help people understand the full range of services available in their community regardless of age, income or disability and help people make informed decisions about obtaining and managing the care options that best meet their needs through public and private payment means.
