ServiceLink of Grafton County is one of ten Aging and Disability Resource Centers located across the state and may be reached toll free at 1-866-634-9412.
Each Center offers Medicare Counseling, Caregiver Counseling, Long-term Care Options Counseling and general information, referral and assistance for services in Grafton County. The Centers are free, accessible and open to the public. Through counseling and education, we help people make informed decisions about obtaining and managing the care options that best meet their needs whether through public or private resources. Powerful Tools for Caregivers classes and caregiver support groups are offered throughout the county. Call or go to nhcaregiverconnections.org for class and event schedules, as well as volunteer training and management resources.
